FORM Mattress Review

FORM Sleep is a Scottsdale, Arizona direct-to-consumer brand (FORM Sleep LLC). It sells four USA-made GelGrid mattresses: Core, Core Hybrid, Prime, and Prime X. Prices run from $1,149 to $3,599, with a 365-night trial and a lifetime warranty. You'll like it if you sleep hot or share a bed. There are caveats. It's online-only, every model has one firmness, and you won't find many independent reviews. You can't try one in a store before you buy.

Checked September 2026 against the brand's own pages and public listings. We haven't sleep-tested these mattresses, so treat this as a buyer's summary of what the brand publishes.

Which mattress suits your sleep needs?

If you sleep hot, all four models share GelGrid airflow and the AeroWeave cover. Testers scored cooling 9/10 on the Core, Core Hybrid, and Prime, and 10/10 on the Prime X. Pick the Prime X if overheating is your main problem. The Core Hybrid is the value cooling pick.

Side sleepers should look at the Prime and Prime X. Their Euro-top, and the Talalay latex on the Prime X, give the most shoulder and hip give. Sleep Junkie measured about 4.5/10 at the Prime's shoulder zone. Back sleepers can try the Prime, whose seven-zone ZoneCoil lumbar support is what the brand pushes hardest. Combination sleepers can try the Core Hybrid or Prime, both scored 10/10 for responsiveness. Couples will like the hybrids, scored 10/10 for motion isolation and edge support.

Stomach sleepers: the Core is the firmest-labeled option. If you need a truly unyielding surface, test it during the 365-night trial. Heavy sleepers should choose a hybrid over the all-foam Core. Eachnight cautions that sleepers over about 230 to 250 lb may want more support than the Prime gives. For budget, kids, and guest rooms, the Core in Twin, Twin XL, or Full works. Prime X is the only model with Talalay latex. If you can't decide, the brand has a simple guide. It suggests Core for budget and Core Hybrid for most sleepers. Prime is for back pain and luxury, and Prime X is the top of the line.

Mattress prices and value

These prices were current in September 2026, under a standing $400-off promotion. Each model's from price is $400 below its list price. Core costs $1,149 Twin and Twin XL, $1,299 Full, $1,399 Queen, and $1,899 King and Cal King, listed from $1,549. Core Hybrid costs $1,549 Twin and Twin XL, $1,799 Full, $1,899 Queen, and $2,499 King and Cal King, listed from $1,949. Prime costs $2,199 Twin and Twin XL, $2,399 Full, $2,599 Queen, and $3,199 King and Cal King, listed from $2,599. Prime X costs $2,399 Twin and Twin XL, $2,799 Full, $2,999 Queen, and $3,599 King and Cal King, listed from $2,799.

Per-size prices come from the site's product data, because the pages show only a from figure. Split King 2-packs are specified but not priced. The brand's own table, dated August 10, 2026, puts the Core Hybrid Queen from $1,549 against the Purple Restore Hybrid Queen from $2,699. Its trial table lists FORM at 365 nights with a lifetime warranty and free pickup. Purple and Casper offer 100 nights and 10 years. Tempur-Pedic offers 90 nights and 10 years, with a $175 charge and required repackaging. Saatva offers 365 nights and a lifetime warranty with a $99 transportation charge.

Independent testers gave all four models 8/10 for value at Sleep Junkie, and Eachnight gave the Core and Core Hybrid 8/10. Both scored the 365-night trial 10/10. The brand doesn't publish bundles, member pricing, or accessory discounts.

What are the mattresses made of?

Every model starts with the same top stack. The cover is AeroWeave cool-touch knit, 98% polyester and 2% spandex, with a non-slip bottom panel of 74% polyester and 26% acrylic latex. The Prime and Prime X add a quilted Euro-top panel sewn in, 2 in on the Prime X. The comfort layer is 1.5 in of GelGrid, a hyper-elastic gel polymer reinforced with Integra-Mesh and finished with a moisture-wicking treatment. The brand says it has 1,400+ open-air channels.

Below that, the models differ. ContourFlex adaptive foam is 2 in on the Core, 1.5 in on the Core Hybrid, 3 in on the Prime, and 1 in on the Prime X. Weights run 37 lb Twin to 79 lb King on the Core, though the FAQ quotes 104 lb for a Queen. The Core Hybrid runs 60 to 129 lb, the Prime 68 to 145 lb, and the Prime X 86 to 183 lb. The Core has 6 in of StableCore high-density foam and no coils. The three hybrids use 8 in of ZoneCoil 7-Series pocketed coils. They have seven zones and a foam-encased perimeter, with 600 coils in a Queen. Under them sits 1 in of BaseFlex foam. The Prime X adds 1 in of ATEX Talalay latex and 1 in of titanium-infused memory foam. The fire barrier is a non-toxic knit that isn't chemically treated, the brand says.

The brand reports CertiPUR-US foams, GREENGUARD Gold, OEKO-TEX Standard 100, and 100% fiberglass-free construction. It says it makes every mattress in its California plant, which is ISO 9001 certified, with 100% quality inspection. For care, you should spot clean only and use a protector to keep the warranty valid. Rotate head to foot every 3 to 6 months, never flip it, and allow 24 to 48 hours for full expansion after White Glove setup. One oddity: the collection page calls the comfort layer FORMFlex grid, while product pages call it GelGrid.

How well do the mattresses hold up?

You won't find long-term durability data yet, because the brand is young. Its Terms and Privacy pages were last updated February 11, 2026, and its comparison table is dated August 10, 2026. The site doesn't publish sagging tests or years-in-use studies. The brand points to its lifetime warranty, its ISO 9001 factory, CertiPUR-US foams, and fiberglass-free build. Reviewers describe Talalay latex as roughly a 20-year material. Integra-Mesh is said to stop the grid from stretching, and the coils are individually wrapped.

Eachnight ran repeated loading cycles on the Prime. It found no meaningful permanent indentation, and the grid returned to shape each time. Its Prime X review notes two handles that make rotating the 15 in, 183 lb King practical. Sleep Junkie calls the builds durable but notes the Core is all-foam and petroleum-based.

Care rules affect lifespan. Rotate every 3 to 6 months, never flip, and use a protector, because stains void the warranty. You'll also need a supportive foundation or adjustable base. The brand's documents disagree on sagging. The FAQ and homepage say over 1.5 in is covered, but the Terms say over 1 in. Treat any 10-year-plus lifespan claim as unproven.

Customer reviews and complaints

Customer reviews are essentially absent. The product pages have an empty review section as of September 2026, so no star rating or count appears on formsleep.com. We found no Trustpilot profile, no Better Business Bureau profile, and no Amazon listing. SmartCustomer, formerly Sitejabber, lists formsleep.com with 0 reviews.

Expert coverage does exist. Sleep Junkie scored the Core 9/10 and the Core Hybrid, Prime, and Prime X 10/10. Eachnight scored the Core 9/10 and the other three 10/10. PureWow scored the Core Hybrid 97/100, with 20/20 for value and quality. Weigh the conflicts. Dr. Jordan Burns wrote all four Eachnight reviews and is also the expert whose 5.0 endorsement FORM displays. Eachnight and Sleep Junkie run affiliate links and ads, and Sleep Junkie is owned by Healthy Sleep LLC. PureWow discloses gifted items and affiliate links.

Published criticisms vary. PureWow's tester said the firmness takes getting used to, and unboxing was cumbersome. Sleep Junkie flagged edge compression on the Core and its petroleum-based foams. It also warns stomach sleepers who need a very firm surface to test carefully. Eachnight says sleepers over roughly 230 to 250 lb may want more support. Its Prime cooling dropped to 9/10 when two people used the bed in a hot room. We found no Reddit threads, third-party YouTube reviews, or press coverage.

Where can you try and buy?

FORM sells online only. You'll find all four mattresses at formsleep.com. There are no showrooms, no store locator, and no third-party retailers as of September 2026. The try-before-you-buy route is the 365-night at-home trial.

Support runs 7 days a week. You can call 888-533-3842, Monday to Friday 8am to 9pm MST and Saturday to Sunday 9am to 6pm MST. There's also live chat, email, and a 24/7 virtual assistant. The brand quotes an average response under 2 hours, and contact-form replies within 2 to 4 hours in business hours. The company is FORM Sleep LLC, 7167 E Rancho Vista Dr, Ste 137, Scottsdale, AZ 85251.

A wholesale program invites mattress, furniture, and regional retailers, designers, wellness clinics, and corporate buyers to apply at formsleep.com/wholesale. No approved stockists are listed. You'll find the brand on Instagram @form.sleep, TikTok @formsleep, and YouTube @formsleep. The site has order tracking and a self-serve return form.

Delivery, setup and removal

Shipping is free to the contiguous United States. For Alaska and Hawaii, you'll need to contact support for options and rates. The Core and Core Hybrid arrive compressed in a box by FedEx Ground. Most orders ship in 5 to 7 business days, though one FAQ passage says 3 to 8. You unpack and place them yourself.

The Prime and Prime X include free White Glove delivery. A carrier contacts you to set an appointment, brings the mattress in, sets it up, and can haul away your old one. The brand asks you to allow 2 to 3 weeks for scheduling. They ship flat-boxed, for example 61 x 81 x 16 in for a Queen Prime. Allow 24 to 48 hours after setup for full expansion.

Returns reverse the process. The brand arranges free pickup and doesn't require repackaging. A tracking page shows delivery status. The brand doesn't publish in-store pickup, expedited delivery, or delivery fees.

Sleep trial and returns

The trial lasts 365 nights, starting on the delivery date. You must keep the mattress for a 30-night minimum, because the brand says your body needs 2 to 4 weeks to adapt. After that, you can return it any time inside the 365 days. Returns are free. You contact the sleep team and they schedule pickup. You don't need the box or shrink wrap.

Refunds are full, minus the value of any non-returnable promotional gifts. They go to your original payment method within 5 to 10 business days after pickup. Shipping charges, if you paid any, aren't refundable. There are no restocking fees. The mattress must be usable, without misuse damage or stains that void the warranty. You must start the return before the trial ends.

Pillows, sheets, protectors, frames, adjustable bases, final-sale items, and gift cards can't be returned under the trial. As of September 2026, none were listed for sale. The FAQ tells buyers who dislike the single Medium firmness to exchange or return within 365 days. The brand doesn't publish an exchange process, fee, or timeline. Full terms are in the Terms and Conditions, last updated February 11, 2026.

Warranty

Every FORM mattress has a Lifetime Warranty, detailed in section 8 of the Terms and summarized in the FAQ. It covers defects in materials and workmanship for the mattress's useful life, from the original purchase date. It's non-transferable, so only the original buyer is covered. Covered are visible indentations over 1 in that an unsupportive foundation didn't cause. Foam that splits or cracks under normal use is covered. So are cover defects like seam separation and zipper failure.

The FAQ and homepage say over 1.5 in. The Terms say 1 in, which is stricter. Not covered are normal wear, comfort or firmness preference, misuse, stains, damage from a bad foundation, bending or folding, burns, cuts, liquid, and abuse.

To claim, you contact support with your order number, photos, and a description. The site also offers warranty registration to activate coverage. The brand doesn't publish a proration schedule, who pays shipping, or a repair-versus-replace policy. The Terms and FAQ are the only warranty sources.

FORM vs. other brands

FORM positions itself against Purple and Tempur-Pedic, with Casper and Saatva in its trial comparison. The core argument is grid versus foam. Its tables contrast GelGrid with Integra-Mesh against Purple's GelFlex Grid and Tempur-Pedic's memory foam. It claims a steadier grid feel, open-channel cooling, a 365-night trial against Purple's 100 and Tempur's 90, and a lifetime warranty against 10 years for both.

On price, the brand's table dated August 10, 2026 shows the Core Hybrid Queen from $1,549 against the Purple Restore Hybrid Queen from $2,699. That's a $1,150 gap. Saatva matches 365 nights and a lifetime warranty but charges $99 transportation. Purple and Casper offer 100 nights with 10-year warranties. Tempur-Pedic charges $175 and requires repackaging. FORM stands out with fiberglass-free construction, CertiPUR-US, GREENGUARD Gold, and OEKO-TEX certifications, US manufacturing, and free two-way shipping.

Where FORM lags, you should know it's young and small, with no long-term history or store presence. All four models share one Medium firmness. It has no organic option except the Prime X's 1 in Talalay layer, no adjustable air technology, no accessories, and no customer review base. It has far fewer independent reviews than Purple, Tempur-Pedic, Casper, or Saatva. Its most visible reviewer is also its featured endorser, so discount that.

How we put this review together

We read the brand's own product, policy and warranty pages, then checked retailer listings and public complaint sites like the Better Business Bureau. We last checked everything here in September 2026. Prices and policies change, so confirm them on the seller's page before you buy. You can read more in our methodology and editorial policy.

Quick answers before you buy

What firmness do they come in?

The FAQ says all four models are a single Medium feel, so you don't get a choice at checkout. The collection page labels Core Firm, Core Hybrid Medium-Firm, Prime Medium, and Prime X Medium Firm. Testers measured a zoned feel of roughly 4.5 to 5 out of 10 at the shoulder and about 7 at the lumbar zone.

Can I try one in a store?
